What Are The Requirements To Apply For A Medical Card In Illinois?


1) You must be able to provide proof of your Illinois residency 2) You cannot have a Commercial Driver’s License (CDL) 3) You must have a diagnosis that qualifies in Illinois 4) You must submit a signed Physician Certification with your IDPH application – and that is where we come in! We will provide the Physician Certification and then you can apply to get your card.

How Much Does the Process Cost?


1) Physician Appointment Fee – due after the online video chat with the physician. If you qualify for a medical card we charge $249. If you don't qualify you don't pay anything. 2) IDPH Application Fee – due when you complete the IDPH online application $100 per year, $200 for 2 years, $250 for a three year card 50% off for veterans and people on disability
